Checkup, HBV (Hepatitis B virus) vaccine, if not given before baby leaves the hospital. |
2 Months |
Checkup, DTaP (Diphtheria, Tetanus & accellular Pertussis), Polio, HIB (Hemophilus Influenza B meningitis) and Pneumococcal vaccines. |
4 Months |
Checkup, DTaP, Polio, HIB and Pneumococcal vaccines. |
6 Months |
Checkup, DTaP, HBV, HIB and Pneumococcal vaccines. |
9 Months |
Checkup, HBV vaccine, Hemoglobin (anemia test), Polio Vaccine. |
12 Months |
Checkup, MMR (Mumps, Measles & Rubella), Pneumococcal vaccine. |
15 Months |
Checkup, DTaP, HIB and Chickpox vaccines. |
18 Months |
Checkup. |
2 – 18 Years |
Annual checkups. DTaP, Polio and MMR at 5 years of age. Tetanus/Pertussis at 11 years of age. Meningitis at 12, 15, 17-18 years of age. Human Papaloma Vaccine (HPV) at 11-12 years of age. |
Changes may be made as revisions occur in the immunization schedule as recommended by the American Academy of Pediatrics.
